Consultancy and Advice 

 The Senior Medical and Scientific staff of Microbiology laboratory  provides expert advice on clinical, scientific and technical microbiological issues to users of the service.

The clinical service is consultant-led and available on a 24-hour basis to hospital clinicians and GPs.

Microbial Epidemiology and Surveillance

The Microbiology laboratory complies with national and local requirements for the surveillance of infectious diseases and reporting to the designated agency. Microbiology laboratory  continues to monitor all organisms of public health significance. Antimicrobial resistance patterns are reviewed as required by the HPA and other national organisations. This information is held on a database, the use of which is available subject to the requirements of the Caldicott report.

Outbreak support

Support to the HPA Health Protection Units, CsCDC and Local Authority Environmental Health Departments is provided at two levels. Consultant staff provide expert clinical advice on specific infections and guidance on appropriate laboratory investigations. They also act as the local or regional microbiology co-ordinator. The Microbiology laboratory provides laboratory support for testing of specimens collected in the course of outbreak investigations. Staff are co-opted onto Outbreak Control Teams and attend outbreak review meetings. Senior staff are also expected to give advice and support to the work of other Agencies.

Emergency response

The Microbiology laboratory  provides laboratory support in the event of microbiological emergencies, senior staff contribute to local and regional response plans.

Provision of expert advice

The Microbiology laboratory is a source of expert advice on Public Health microbiology, microbial epidemiology and outbreak control issues. This advice is available to colleagues within the wider HPA, Local Authority representatives, NHS Trusts and to other health professionals.

A wide range of molecular diagnostic assays are provided, using genome amplification techniques to detect specific infections. These include viral load monitoring for blood-borne infections, such as HIV, HBV, HCV and CMV to measure response to drug therapy. In addtion, all tests traditionally carried out by Virus Isolation methods have been transferred to Molecular techniques, providing rapid and definative results for a range of acute viral infections.

Screening tests to help manage outbreak situations, such as Influenza, RSV and Norovirus are also carried out by this laboratory.

Antiviral Resistance Testing Service (ARTS)

Changes to Chlamydia Service

 

The ARTS is a national specialist unit. Assays are undertaken to determine the susceptibility of Human Immunodeficiency Virus (HIV), Hepatitis B virus (HBV), Cytomegalovirus (CMV)

One of the key functions of ARTS is the collection of evidence relating to the role of drug resistance in clinical management and outcomes.

As part of an MRC surveillance programme on recent HIV seroconversions, HIV genotyping is also performed.

This unit is part of a network of four laboratories using the latest techniques to provide mycobacterial detection, identification and susceptibility testing for England and Wales. Data obtained contributes to national and local surveillance programmes.

At Birmingham, the RCM offers primary testing for the Heart of England NHS Foundation Trust as well as a number of other local Trusts. Isolates of mycobacteria are received from 39 other laboratories throughout the West and East Midlands, for full identification, antibiotic susceptibility testing and epidemiological typing.

This unit provides microbiological support to a number of West Midlands Local Authority Environmental Health departments, as well as the host Trust. It is UKAS accredited and an EU Official testing laboratory.

The laboratory participates fully in local, national and European Union-wide surveillance programmes and has an important role in the investigation of food and water-borne outbreaks.

A number of staff are recognised as food examiners under the terms of the Food Safety Act 1990 and are available to advise environmental health and industry colleagues on FWE microbiology.
